* Consent to treatment of minor - notarized. download *

* Permission for minor child to enter the Republic of Mexico - notarized. download *

* Certificate of accident and health insurance. download *

* Photocopy of passport

* Bring the official passport to "final departure meeting"

* Scholarship Application (if applicable) - contact George Barks for more information.

Don't have a passport? Obtain passport forms at any US Post Office or download here.

Items required for a passport:
- Passport application; Both parents must be present with photo ID and the child when applying for a passport or provide a notarized statement of consent for the absentee parent.
- Original certified birth certificate - not a photo copy or abstract
- 2 photos - passport size
- Both parents must sign the passport application in front of the passport agent
